Cancellation Policy:
If you do not show up for your scheduled therapy appointment, and you have not provided notification at least 24 hours in advance, you will be required to pay the full cost of the session.

Services
LETTERS
​
Therapists do not provide disability letters or letters regarding your ability to work, or any letters that would inform providers of your mental health history. You must discuss these needs with your medical or primary care provider.
A signed Release of Information is required for any communication between therapist and outside parties.
Support letter: If a letter is required attesting the client’s needs the therapist will provide it for a fee of $50 per one-page letter and $25 for each additional page. Letters are only provided to clients who have been seen for 8 sessions or longer.
Workplace accommodations letter: Letter of support for allowable accommodations in the workplace when such letter is determined to be necessary and appropriate. Letter is released to client only.

Emotional Support Animal: Therapist is able to provide Emotional Support Animal letters for housing and flying if a therapeutic relationship has been established for at least 30 days and at least 4 sessions. The letter fee is $100 for a single letter.
Record requests: For record requests, the retrieval fee is a flat fee of $50 with an additional $2 per page.

COURT APPEARANCE

Court Appearance Clients are discouraged from having the therapist subpoenaed. Though the clients attorney, who initiates the subpoena request is responsible for the court appearance and testimony fees, it does not mean that the therapist's testimony will be solely in in the client's favor. The Therapist will only testify their professional opinion and to the facts of the case.
The following fees apply for court appearances:
Preparation time (including submission of records) $200/hour
Phone calls $200/hour
Email or written letters $200/hour
Time required in giving testimony $400/hour
Filing a document with the court $100 (Plus court fees)
